Il glioblastoma multiforme

Foto di fioreSi tratta di una patologia a pertinenza cerebrale di notevole aggressività e raramente suscettibile di asportazione chirurgica completa.

Le cellule che lo costituiscono hanno uno scarso grado di differenziazione e crescono in modo disordinato tanto che spessissimo l'aspetto radiologico è tipico con aree necrotiche ed infartuali nel contesto della massa tumorale.
Il trattamento, oltre una riduzione od asportazione chirurgica (ripeto, assai raramente radicale) si avvale dei presidi terapeutici rappresentati da chemioterapia, combinata a radioterapia ed ipertermia.

I chemioterapici dotati di maggiore efficacia in questa malattia sono la temozolomide e la fotemustina. La temozolomide ha una modalità di assunzione orale (trattandosi di capsule) mentre la fotemustina si somministra per via endovenosa. Le moderne apparecchiature per radioterapia ed ipertermia consentono un trattamento più energico e più conservativo sul parenchima cerebrale sano.

In tutto il mondo la ricerca procede alacremente con il testing di nuovi farmaci (ad esempio il bevacizumab) e con soluzioni che prevedano l'uso delle moderne tecniche legate alle cellule staminali.
